All Scott AFB EFMP program components and the School Liaison are collocated to best meet your EFMP needs.  We are located at the Military & Family Readiness Center conveniently next to the Education Center, Post Office, Base Auditorium, Shoppette, and Burger King.     

The M&FRC is open to walk-in customers Monday-Friday from 0700-1600.  A member of the M&FRC team is available during those hours to assist with setting up appointments.

You can schedule an appointment by calling 618-256-8668 or emailing 375FSS.EFMP@US.AF.MIL.  Please be advised that there could be long waiting times during our walk-in hours as we work with individual families.

Family Needs Assessment

EFMP Family Support can meet with you any time to complete a family needs assessment.  The assessment is person-centered, family-focused, and strength-based to support the entire family with services and resources that best meet their needs.  For resource and referral or to complete a family needs assessment with service plan email or call us at 618-256-4746 or 375fss.efmp@us.af.mil.

Family Needs Assessment

Per 10 USC Statute 1781, Section 563, Q-coded Airmen must complete or update a Family Needs Assessment/Service Plan with the local EFMP Family Support (FS) Coordinator NLT 60 days prior to a permanent change of station CONUS or OCONUS. An outlook calendar invitation is being sent to you to schedule this mandatory appointment.

We encourage but do not require your spouse and/or family member/s to attend. This is especially important if those family members manage appointments and support services for themselves or the EFMP family member.

Illinois Waiver Services  

Illinois Waiver Services - Prioritization for the Needs of Services (PUNS)

PUNS is a database that registers individuals who want or need Developmental Disability Waiver services funded by the IL Department of Human Services.  Contact your area service coordination agency. PUNS enrollment will help communicate a possible need for services once your child turns 22.  It is recommended to enroll at any age as the state uses this data to track future needs.

 Air Force Aid Special Needs Grant

The Air Force Aid Special Needs Grants assists eligible Airmen/Guardians who are faced with increased financial requirements due to costs associated with having dependents with special needs. Eligibility:  Active duty, Active Guard Reserve (AGR), Guard/Reserve on an Initial or Continuation Full-time order, and Traditional Guard/Reserve on a long-term order who have dependents enrolled in the Air Force's Exceptional Family Member Program (EFMP).

Grant Amount:  Up to $3000 per special needs family member over a five-year period.

This accelerated order is ONLY for scheduling household goods, passenger travel, and processing passport/visa applications as required.  Members/member family are not authorized to depart current duty station without an authenticated PCS order amendment documenting confirmation of the dependent travel screening clearance.  No financial obligations that may cause hardship should be made until the final set of orders are received.

EFMP Medical Forms 

If you are PCSing with dependents, please go to MyVector.  You will need to complete a 2 to 12 question screener (length depends on the answers given).  If additional information is needed, myVector will provide a link to the Family Member Relocation Clearance. Airman and Guardians who have dependents will recieve a MyVector system-generated email up to fifteen (15) days after the Assignment Creation Date (ACD) and are within eight (8) months of their Report-No-Later-Than-Date (RNLTD). If the member is relocating to a CONUS location, answers "No" to all questions, and has no family members enrolled in EFMP, then the member is cleared to receive PCS orders.  

If a member is enrolled in EFMP and answers "Yes" to one of the questions, the member will receive two (2) notifications:  the "Additional Screening Required" and the requirements to complete the FMTS application.

All family members who are traveling to an overseas location MUST have a complete review of their medical records.  A "comprehensive exam" within the past 24 months must be in the family member's medical chart. If a member is relocating to an OCONUS Geographically Separated Unit (GSU) or Sister Service locations, and a family member has medical or mental health needs, then a DD Form 2792 is required to be submitted.

To initiate an EFMP enrollment, please use the DD2792-1 and use the DD Form 2792-1 Provider One Pager.  Once complete, send to the EFMP Medical email org box listed above for processing.  

In order to make your clearance run as smoothly as possible, please ensure that records from off base providers have been submitted to the Tricare Patient Administration Office on the second floor of the Medical Group to be added to your on base record.

Dependent Travel Not Approved (DTNA) Process

When a member receives a travel determination of "RED"/DTNA on their FMTS application, the member will receive a MyVector generated email and a MyVector notification within their account.  

Below are the DTNA options provided to the service member:

Reconsideration - Members can request a review of updated/new information to potentially overturn the final travel determination of their FMTS application.

Reassignment - If a member is identified as "Mandatory Move" and the member does not request a reconsideration or the member's reconsideration is "sustained", the member will work with their assignment navigator to provide options to be set tot eh member's assignments functional for consideration.

Deferment - Members can request their assignment to be deferred based on extenuating medical needs.  Member's Assignments Navigator will assist with this option.

Cancellation - Member is identified as "Not a Mandatory Mover" and does not request a reconsideration OR the member's reconsideration has been sustained to the gaining location; their assignment will be cancelled.
